What is the Intellectual Disability (ID) Waiver?

The ID Waiver is a Medicaid program for individuals:

  1. Under 6 and at developmental risk
  2. 6 and older who are diagnosed with an intellectual disability

This waiver offers many services which the individual can receive in their homes and communities.
 

How Do I apply?

Contact your local Community Services Board (CSB) or Behavioral Health Authority .  Request a screening.  The individual will be screened using the “Level of Functioning Survey.” 
  

What Are The Criteria?

You must qualify in two areas:

  1. Financially
  2. Eligible for placement in an intermediate-care facility for persons with mental retardation or other related conditions (ICF-MR) – determined using the “Level of Functioning Survey.”

What are the benefits of being on the ID Waiver?

  1. Consumer or Agency Directed Services:  Companion Services, Personal Care & Respite Care
  2. Many Additional Services:  day support, supported employment, in-home residential support,  residential (group home) support services, therapeutic consultation, skilled nursing services, crisis stabilization and supervision, environmental modifications, assistive technology, personal emergency response system (PERS), and prevocational services.

OK, I’m approved, now what?

  1. Consumer or Agency Directed Services:
    1. Select Model of Service Delivery and Choose Provider for Personal Care, Respite and Companion Care.  Under Consumer Directed (You are the Boss, but a Service Facilitator helps you.)

    1. Service Facilitator conducts Initial Visit
      1. Trains Employer of Record (EOR) on hiring procedures
      2. Initiates process by contacting Medicaid’s Fiscal Agent (PPL)
      3. Conducts Initial Assessment
      4. Creates Plan Of Care
      5. Estimates how many hours the Individual will qualify for
    1. Post Initial Meeting
      1. Service Facilitator – Requests Prior Authorization for hours
      2. EOR is set up to be employer
      3. EOR recruits, hires and schedules attendants.
    1. Begin Services

  1. Work with your case manager regarding additional services.
