EDCD Waiver Process
What is the Elderly or Disabled with Consumer Direction (EDCD) Waiver?
The EDCD Waiver is a Medicaid program for elderly or disabled individuals to recieve services in their homes and communities. These services are consumer directed.
How do I apply?
- Bring a completed financial application for Adult Medicaid to your local Department of Social Services (DSS) and request a screening for Long Term Care.
- Remember if you are filling this out for a child, ONLY PUT THE CHILD’s INFORMATION ON THE FORM. Put income and assets for the child only.
- Write “EDCD/Long Term Care Screening” on the top of the form.
- Print The Application
- Find Your Local DSS

- A social worker from DSS and a nurse from the local Department of Health (DOH) will schedule an assessment with you.
- This will take place in your home.
- The individual must be in attendance.
- They will conduct the assessment using the Uniform Assessment Instrument
- They will be looking for:
- A Disability
- Functional Dependencies
- Medical/Nursing Needs

What are the criteria?
You must qualify in two areas:
- Financially
- Meeting Nursing Home Criteria – determined during your screening.
- Must have functional needs.
- Must have medical/nursing needs. Examples of these are
- Multiple Complex Inter-Related Conditions
- Unable to self assess or monitor
- Respitory Therapy
- Etc.
Ok, I’m approved, now what?
- Select Model of Service Delivery and Choose Provider
- Agency Directed or Consumer Directed (You are the Boss, but a Service Facilitator helps you.)

- Service Facilitator conducts Initial Visit
- Trains Employer of Record (EOR) on hiring procedures
- Initiates process by contacting Medicaid’s Fiscal Agent (PPL)
- Conducts Initial Assessment
- Creates Plan Of Care
- Estimates how many hours the Individual will qualify for
- Post Initial Meeting
- Service Facilitator – Requests Prior Authorization for hours
- EOR is set up to be employer
- EOR recruits, hires and schedules attendants
- Begin Services
