What is the Individual & Family Developmental Disabilities Support (DD or IFDDS) Waiver? |
The DD Waiver is a Medicaid program for individuals over 6 years of age who have a developmental disability. This waiver offers many services which the individual can receive in their homes and communities. These services can be consumer or agency directed.

How Do I apply? |
- Send a completed “Request for Screening” to your local screening facility
|
What Are The Criteria? |
You must qualify in two areas:
- Financially
- Level Of Care need – determined during your screening.

What are the benefits of being on the DD Waiver?
- Consumer or Agency Directed Services: Companion Services, Personal Care & Respite Care
- Many additional Services: day support, supported employment, in-home residential support, therapeutic consultation, skilled nursing services, family and caregiver training, crisis supervision, environmental modifications, assistive technology, personal emergency response system (PERS), and prevocational services.

OK, I’m approved, now what? |
- Consumer or Agency Directed Services:
- Select Model of Service Delivery and Choose Provider for Personal Care, Respite and Companion Care. Under Consumer Directed (You are the Boss, but a Service Facilitator helps you.)

- Service Facilitator conducts Initial Visit
- Trains Employer of Record (EOR) on hiring procedures
- Initiates process by contacting Medicaid’s Fiscal Agent (PPL)
- Conducts Initial Assessment
- Creates Plan Of Care
- Estimates how many hours the Individual will qualify for
- Post Initial Meeting
- Service Facilitator – Requests Prior Authorization for hours
- EOR is set up to be employer
- EOR recruits, hires and schedules attendants.
- Begin Services
